Objective: The objectives of this study are to understand the current functions, structure and operation of hospital ethics committees (HECs) in Shanghai and to facilitate their improvement. Methods: (1) A questionnaire survey, (2) interviews with secretaries and (3) on-site document reviews of HECs in Shanghai were used in the study, which surveyed 33 hospitals. Results: In Shanghai, 57.56% of the surveyed hospitals established HECs from 1998 to 2005. Most HECs used bioethical review of research involving human subjects as well as (...) bioethical review or consultation regarding medical care services and administrative decision- making. Of the surveyed HECs, 14.3% did not provide any formal bioethical training to the HECs’ members and many HECs had no standard operating procedures. Some HECs had no clear definition of what was “conflict of interest” that should be considered by the HECs, while 44.4% of the HECs did not perform continuing review. Discussion: After the issues of related national regulations, more and more hospitals established HECs in Shanghai, but the functions of HECs need to be further developed and formal training on bioethics should be provided to HEC members. To assure the independence and good performance of HECs, the conflict of interest procedure, the standard operating procedures and bioethical review should be improved. Conclusion: HECs in Shanghai had developed in the preceding 10 years and they played great roles in protecting the rights and welfare of human subjects and patients; some areas need improvement. Intelligent chatbot is an essential application of NLP to allow understanding of users’ utterance and responding in understandable sentences for specific applications simulating human-to-human conversations and interactions for problem solving or Q&As. This research studies emerging technologies for NLP-enabled intelligent chatbot development using a systematic patent analytic approach. Some intelligent text-mining techniques are applied, including document (...) term frequency analysis for key terminology extractions, clustering method for identifying the subdomains, and Latent Dirichlet Allocation for finding the key topics of patent set. In the present research, we qualitatively document the process by which spiritual leader and leadership emerge in religion-based organizations. Data from 26 participants in three religion-based organizations revealed three cardinal themes that depict the development of spiritual leader and spiritual leadership, the process of developing a spiritual leader and spiritual leadership, and outcomes of spiritual leader and leadership development. Based on the results, we propose a model that depicts the phases involved in the development of spiritual leader/leadership in the religion-based (...) workplace. These phases are proposed to impact the outcomes for the leader, followers, and the organization. The implications of our results are discussed. (shrink)

Event-related desynchronization, as a proxy for mirror neuron activity, has been used as a neurophysiological marker for motor execution after mirror visual feedback. Using EEG, this study investigated ERD upon the immediate effects of single-session MVF in unimanual arm movements compared with the ERD effects occurring without a mirror, in two groups: stroke patients with left hemiplegia and their healthy counterparts. During EEG recordings, each group performed one session of mirror therapy training in three task conditions: with a mirror, with (...) no mirror, and with a covered mirror. An asymmetry index was calculated from the subtraction of the event-related spectrum perturbations between the C3 and C4 electrodes located over the sensorimotor cortices contralateral and ipsilateral to the moved arm. Results of the effect of task versus group in contralateral and ipsilateral motor areas showed that there was a significant effect of task condition at the contralateral motor area in the high beta band at C3. High beta ERD showed that the suppression was greater over the contralateral hemisphere than it was over the ipsilateral hemisphere in both study groups. The magnitude of low beta ERD in patients with stroke was more suppressed in contralesional C3 under the no mirror compared to that of the covered mirror and similarly more suppressed in ipsilesional C4 ERD under the no mirror compared to that of the mirror condition. The correlation analysis revealed that the magnitude of ERSP power correlated significantly with arm severity in the low and high beta bands in patients with stroke, and a higher asymmetry index in the low beta band was associated with higher arm functioning under the no-mirror condition. There was a shift in sensorimotor ERD toward the contralateral hemisphere as induced by MVF accompanying unimanual movement in both stroke patients and healthy controls. The use of ERD in the low beta band as a neurophysiological marker to indicate the relationships between the amount of MVF-induced ERD attenuation and motor severity, and the outcome indicator for improving stroke patients’ neuroplasticity in clinical trials using MVF are warranted to be explored in the future. The ordering tendency in binary f -Ti solid solution containing 3sp or 4sp simple-metal or 3d transition-metal solute is investigated systematically by the linear muffin-tin orbital method within the atomic sphere approximation . We demonstrated that the effective pairwise interaction energy in a solid solution is equal to half the solute-solute interaction energies and can be evaluated by a supercell total energy approach. The calculations of EPI energy both with and without volume relaxation of the supercells and local density of (...) states show that the EPI energies of Ti-SM and Ti-TM solutions are dominated by different factors. For Ti-SM solutions, the EPI energies are of large absolute values with a negative sign, indicating strong ordering tendency in these solutions. The volume relaxation does not alter the EPI energy substantially. The calculated LDOS shows that the ordering tendency in Ti-SM solutions may be related to the hybridization between the electrons of the SM atoms when they are close to each other. For most Ti-TM solutions, if calculated without relaxation, the absolute EPI energies are very small; however, if calculated with relaxation, they are of relatively large positive values, indicating a clustering tendency in these solutions. By combining the calculated EPI energy and Flinn's model for short-range order strengthening, the increase in critical shear stress ‰ sro due to SRO is estimated for Ti-SM alloys, and the results qualitatively agree with experiment. In information theory there is a fundamental principle, usually referred to as the informational “uncertainty principle”, which expresses a limitation of any information processing system (or agent) in terms of a relation between the system's response property and its inherent processing capacity. From this principle, it can be argued that a salutary strategy for dealing with conflicting information processing requirements is to adopt various complementary processes (or channels). Donald M. MacKay had attempted to relate the informational uncertainty principle to spatial (...) and temporal response properties of neurons in the mammalian visual cortex, and suggested that the spatial and the temporal aspects of such neurons are complementary. I attempt to extend his efforts and to show that the informational uncertainty principle may indeed underlie many complementary relations exhibited in human perception and cognition, such as the relation between the two principal processing streams in vision and the relation between parallel and serial processes in cognition. In this study, we examine the nature of the relationship between ethical leadership and unethical pro-organizational behavior (UPB), defined as unethical behavior conducted by employees with the aim of benefiting their organization, and whether the strength of the relationship differs between subordinates experiencing high and low identification with supervisor. Based on three-wave survey data obtained from 239 public sector employees in China, we find that ethical leadership has an inverted u-shaped (curvilinear) relationship with UPB. As the level of ethical leadership (...) increases from low to moderate, UPB increases; as the level of ethical leadership increases from moderate to high, UPB decreases. Further, we find that the strength of this inverted u-curve relationship differs between subordinates with high and low identification with supervisor. That is to say, the inverted u-shaped relationship between ethical leadership and UPB was stronger when subordinates experienced high levels of identification with supervisor. The theoretical and managerial implications of our findings for understanding how to manage UPB in an organizational context are discussed. (shrink)

Atualmente, biotecnologias são empregadas na forma de protocolos a fim de sincronizar o crescimento folicular e a ovulação, bem como permitir que vacas atuem como receptoras de embriões visando melhorar o potencial genético do rebanho. O objetivo desse estudo foi avaliar o efeito da utilização de implantes intravaginais de progesterona de 1º, 2º e 3º uso no protocolo de transferência de embriões em tempo fixo (TETF) sobre a taxa de prenhez em receptoras girolandas. O experimento ocorreu em uma fazenda localizada (...) no município de São Sebastião do Maranhão (MG). Foram utilizadas 74 vacas submetidas aos seguintes protocolos: No dia 0 (D0) administrou-se 2 ml de 17β-Estradiol e progesterona e foi colocado o implante intravaginal de progesterona. Estes implantes foram colocados de forma aleatória, sendo de 1° uso (grupo 1; n=15), 2° (grupo 2; n=24) ou 3° (grupo 3; n=35).No D8, os implantes foram retirados e administrou-se 2 ml PGF2α; 1,5 ml de eCG e 1 ml de 17β-Estradiol e progesterona. A inovulação dos embriões in vitroo correu no D (16) para todas as vacas que apresentaram um corpo lúteo. As variáveis qualitativas (prenhe ou não prenhe) foram comparadas em tabelas de contingência e analisadas pelo teste de qui-quadrado a 5% de probabilidade. Neste estudo não houve diferença estatística entre os grupos protocolados em relação ao aproveitamento como receptoras: 1º uso (53,3%), 2º (79,2%) e 3º (68,65). Ademais, observou-se uma taxa de prenhez entre os grupos de 37,5% para o 1º uso, 42,1% para o 2º e 33,3% para o 3º (P>0,05). Dentre as vantagens ao reaproveitamento dos implantes multiusos destaca-se a redução dos custos dos protocolos hormonais, já que a progesterona é o hormônio mais oneroso, desta forma, a maximização do uso reduz o valor final do protocolo sem prejuízos a taxa de prenhez. Smooth function is the essence of the ε-smooth support vector regression (ε-SSVR) which is an unconstrained convex quadratic programming. In this paper, Bézier function as a new smooth function is proposed to replace the ε-insensitive loss function of ε-SSVR with tolerating a smaller error in fitting given data sets linearly and nonlinearly. The ε-Bézier function smooth support vector regression (ε-BSSVR) model is presented. Theoretical analyses show that the derived smooth function has improved approximation precision compared with other smooth approximate functions. (...) A new smooth method for solving the support vector machine classification (SVC) is presented. Since the objective function of the unconstrained SVC is non-smooth, we apply the smooth technique and replace the SVC function with Bézier function and get a class of Bézier smooth support vector machines (BSSVM). The fast Newton-Armijo algorithm is used to solve the BSSVM. Theoretical analysis and numerical results illustrate that this smooth SVM model improves in efficiency and accuracy compared with other smooth methods.

OBJECTIVE: To report and analyse the pattern of end-of-life decision making for terminal Chinese cancer patients. DESIGN: Retrospective descriptive study. SETTING: A cancer clinical trials unit in a large teaching hospital. PATIENTS: From April 1992 to August 1997, 177 consecutive deaths of cancer clinical trial patients were studied. MAIN MEASUREMENT: Basic demographic data, patient status at the time of signing a DNR consent, or at the moment of returning home to die are documented, and circumstances surrounding these events evaluated. RESULTS: (...) DNR orders were written for 64.4% of patients. Patients in pain (odds ratio 0.45, 95% CI 0.22-0.89), especially if requiring opioid analgesia (odds ratio 0.40, 95% CI 0.21-0.77), were factors associated with a higher probability of such an order. Thirty-five patients were taken home to die, a more likely occurrence if the patient was over 75 years (odds ratio 0.12, 95% CI 0.04-0.34), had children (odds ratio 0.14, 95% CI 0.02-0.79), had Taiwanese as a first language (odds ratio 6.74, 95% CI 3.04-14.93), or was unable to intake orally (odds ratio 2.73, 95% CI 1.26-5.92). CPR was performed in 30 patients, none survived to discharge. CONCLUSIONS: DNR orders are instituted in a large proportion of dying Chinese cancer patients in a cancer centre, however, the order is seldom signed by the patient personally. This study also illustrates that as many as 20% of dying patients are taken home to die, in accordance with local custom. (shrink)

This article puts forward a novel class of Bézier smooth semi-supervised support vector machines(BS4VMs) for classification. As is well known, semi-supervised support vector machine is introduced for dealing with quantities of unlabeled data in the real world. Labeled data is utilized to train the algorithm and then adapting it to classify the unlabeled data. However, the objective semi-supervised function is not differentiable globally. It is required to endure heavy burden in solving two quadratic programming problems with inversion matrix operation. To (...) solve this problem, the interpolation Bézier functions for replacing the non-smooth term have been investigated. Firstly, the development of the novel BS4VMs classifier is introduced. Then, the non-linear model and convergence analysis are presented. To show how the BS4VMs can be practically implemented, experiments on synthetic and real data sets are offered. The theoretical analysis and experiments comparisons clearly illustrate the BS4VMs are superior to the proposed methods in both cross-validation classification accuracy and calculating time. ABSTRACT:Regulatory focus theory is proposed as offering an explanation for the influence of ethical leadership on organizational citizenship behaviors and employee commitments. The prevention focus mindset of an employee is argued to be the mechanism by which an ethical leader influences extra-role compliance behavior as well as normative commitment, whereas the promotion focus mindset of an employee is argued to be the mechanism by which an ethical leader influences extra-role voice behavior as well as affective commitment. Moreover, leader-member exchange is (...) proposed as a moderator of the relationship of ethical leadership to regulatory focus mindsets and employee behavior and commitments. Using the data collected in two waves from 250 working adults, we tested the proposed relationships with moderated mediation bootstrap procedures. The findings generally support the hypothesized relationships and point toward important implications for ethical leadership in work settings. (shrink)

Although accumulating evidence suggests the COVID-19 pandemic is associated with costs in mental health, the development of students' mental health, including the change from their previous levels of depression and anxiety and the factors associated with this change, has not been well-studied. The present study investigates changes in students' anxiety and depression from before the pandemic to during the lockdown and identifies factors that are associated with these changes. 14,769 university students participated in a longitudinal study with two time points (...) with a 6-month interval. Students completed the Anxiety and Depression subscales of the Symptom Checklist 90 before the COVID-19 outbreak, and the Self-rating Anxiety Scale and Self-rating Depression Scale during the pandemic. The prevalence of anxiety and depression symptoms were 1.44 and 1.46% at Time 1, and 4.06 and 22.09% at Time 2, respectively, showing a 181.94% increase in anxiety and a 1413.01% increase in depression. Furthermore, the increases in anxiety and depression from pre-pandemic levels were associated with students' gender and the severity of the pandemic in the province where they resided. This study contributes to the gap in knowledge regarding changes in students' mental health in response to the pandemic and the role of local factors in these changes. Implications for gender and the Typhoon Eye effect are discussed. (shrink)
